Case Manager
Under the supervision of the Administrator, the Case Manager is responsible for the coordination of Home and Community Base Services (HCBS) provided to clients within the customized living (CL) and Community Access for Disability Inclusion (CADI) waiver programs or clients eligible for Group Residential Housing (GRH) Supplemental Services.
Job Duties
Assist clients on a waiver to access needed services and resources. Complete and monitor the CL tool or CADI assessment on admission, at 14 days, and as needed with a change in condition, but not to exceed every 90 days. Evaluate and monitor services being provided, the resident service plan, and the rate of reimbursement based on assessed Case Mix Index (CM). Document and resolve any discrepancies between services being provided and the assigned case mix, and the rate of reimbursement. Monitor the provision of care through observation of services being delivered, discussion with the direct care team, and review of the electronic health record. Capture changes in service needs (improvement or decline), ensure service needs are accurately reflected in the client's service plan and communicated to the county, managed care organization (MCO) or the contracted case manager. Ensure good communication and acts in a liaison role among admin, business office, DON and nursing staff and other key team members to ensure care levels and billing / receivable are aligned. Develop strong, positive relationships with government officials, MCOs, contracted agencies, through regular communication and public relations strategies. Promote patient-centered coordination of care.
Education and Experience required
The Case Manager must meet the requirements established by state and federal regulations and must have two (2) + years of experience with waiver services and care coordination. RN or LPN licensed in Minnesota in good standing and ability to secure licensure in State(s) where he/she will be supporting.
