The person serving in the position of Referral Specialist at Family Medical Center is responsible to coordinate the care of all patients following the Physician orders post clinic appointment. The Referral Specialist manages the communication and entire handoff/care coordination process including scheduling the patient for internal CHI and outside referrals, assisting with any necessary prior authorizations and patient facilitation regarding coordination of care between providers. This position is directly responsible to assist in keeping Physicians/Providers/staff current of services available in the CHI network as well as keeping providers/staff current in the health plan networks, keeping current in the knowledge of multiple health plans and working with payers, each which has their own criteria. The Referral Specialist completes required documentation accurately/efficiently, and coordinates records with the goal of the coordination process being to assure the patients get to the right place at the right time for the right care. This position reports directly to the business manager.


1. Education and Training (required and preferred): Working knowledge of the health care environment with excellent communication skills. Strong computer skills and medical terminology knowledge required. HS diploma and clinic experience acceptable with secondary education in a health related field preferred.

2. Experience (required and preferred): Minimum of one year clinic or related experience.
